Wraparound Support is provided for students with low incidence disabilities and students designated as Tier 3 using GNETS triangulation matrix during the Fall or Spring semester. Wraparound support strives to help families discover current unmet needs and challenges that are significantly impacting the quality of their life and the student’s school performance. Through a team-based planning and implementation process, the GNETS Community/Parent Liaison works with families in designing a unique set of actions, services, and supports that draw on the family’s strengths to address those needs and challenges, and help the family experience concrete, positive, and measurable improvements in their lives. The GNETS Community/Parent Liaison facilitates Child Family Team meetings (CFTMs)to collaborate with the student, family, and other stakeholders in the student’s life to develop and implement a Wraparound Action Plan that addresses identified needs and challenges. CFTMs are also held to assess the effectiveness of the Wraparound Action Plan, measure its impact, and modify it as needed to maximize its effectiveness.
